Thank you for your question. You've submitted a single photo and you're
19-years-old and you're concerned about the appearance of your left eye. You describe
multiple folds and the appearance and you want to know if there is a non-surgical solution.
Well, in the absence of a full exam, I would just say that I can certainly give you some
guidance on what my impressions are based on this single photo. A little bit of background,
I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial plastic and
reconstructive surgeon. I've been in practice in Manhattan and Long Island for over 20 years
and my initial impression by looking at this photo is that you have a condition called
ptosis. Now ptosis specifically relates to the eyelid and is often referred to as eyelid
ptosis. And in the greater likelihood that you've always had this appearance, it would
be most likely something called congenital ptosis. Now if it is newly acquired or acquired
ptosis, well then I would certainly advise you to very quickly seek a physician for examination
because that is unusual for somebody your age to suddenly have ptosis. But assuming
that it is something that you've always had and may have gotten a little bit worse,
I'll explain to you about what congenital ptosis is and what the solutions are.
To begin with, ptosis means droop and that eyelid drooping is due to a maldevelopment
or lack of proper development of a muscle called the levator muscle. Levator like the
word elevator is the muscle that lifts the eyelid. Now this muscle, when it doesn't
develop properly, it doesn't work as well as a normal muscle. And what does a normal
muscle do? It contracts and it relaxes so therefore this muscle will neither contract
strongly as a normal muscle nor will it relax as a normal muscle. Now when I do a ptosis
evaluation, it is a fairly involved examination. It's basically taking measurements of the
height of the eyelid, the amount of movement of the eyelid and other ophthalmologic indications
of the degree of eye function and just to get a basic understanding as to how significant
the ptosis is relative to how much it blocks your vision as well as to the degree of the
action.
Now you ask if there is a non-surgical solution and I would say simply that there isn't.
In the past, in the distant past, people used to try wearing glasses and have something
that is called eyelid crutch so that the eyelid will be pushed up with the glasses. Those
tend to not to work very well. That's the only non-surgical solution that isn't even
worth mentioning. There are situations where sometimes an eye drop for the purpose of pictures
can cause contraction of another muscle in the eyelid called the Mueller's muscle so
that the eyelid elevates slightly. That might be good for taking photos but in terms of
the more definitive solution, you are talking about a certain kind of surgery.
Now typically surgery for a muscle which has reasonable function is a procedure called
levator resection which is the muscle being shortened so that the eyelid is elevated and
then the alternative if the eyelid muscle doesn't work very well at all and the eyelid
is very droopy, there is a procedure called frontalis sling. And typically people understand
this because when the eyelid is drooping, they tend to raise their eyebrows to try to
keep their eye open. Well that surgery and that muscle that raises the eyebrows is called
the frontalis muscle. So what we do surgically is we attach the muscle, the frontalis muscle
to the eyelid using a variety of different materials, everything from your own fascia
which is from your leg to banked fascia as well as other alternatives such as silicone
and GoreTex™.
Now these are all determined based on evaluation consultation. So I would suggest that you
meet with an oculoplastic surgeon and learn about your options. I would also encourage
you to actually see an ophthalmologist. Get a proper eye exam and determine if there are
any other visual issues that can also be managed. Sometimes people, when they have congenital
ptosis, if it blocks their vision for an extended period of time when they are very young between
0 and 6, they can actually have had a condition called amblyopia where the eve vision doesn't
fully develop. So it is important that at least a proper eye exam will be done before
you undergo any eyelid surgery and then you can actually have more comprehensive understanding
of the eye health as well as the options for correction of the eyelid drooping.
So I am sorry that there isn't a non-surgical option but I think that it's important that
you understand the condition as well as what you should do to pursue the information you
need to make an informed decision. So again meet with an ophthalmologist, an eye doctor,
an MD who can evaluate your vision, evaluate your eye health and then an oculoplastic surgeon
who's an ophthalmologist, who specializes in additional training in plastic surgery
in the eyes and face.

Last week we discussed the struggle between the desires of our flesh to sin and the desire
of the Spirit to do God's will.
This is a constant struggle for the Christian, one that seems impossible at times, and one
that will leave you wanting to give up…which is why as we start chapter 6 of our study
in Galatians today, Paul gives us this encouragement:
"Do not be deceived: God cannot be mocked.
A man reaps what he sows.
Whoever sows to please their flesh, from the flesh will reap destruction; whoever sows
to please the Spirit, from the Spirit will reap eternal life.
Let us not become weary in doing good, for at the proper time we will reap a harvest
if we do not give up."
Galatians 6:7-9 (NIV) If we endure and don't give up, we will reap
a harvest of the fruits of the Spirit we discussed last week.
As we continue to walk in the Spirit, the fruits will be a guaranteed byproduct.
But it's still a struggle.
We will want to give up because it's so much easier to operate in the works of the flesh
than to trust God for the fruits of the Spirit.
A few videos ago we talked about Abraham, who God promised would be the father of many
nations.
We learned it was his faith that lead to his righteousness.
We also learned he had his moments of leaning on his own strength as well.
But overall, he did not give up on trusting in God, and he had one son that, as God promised,
lead to many nations.
It was his faith that lead to his harvest.
Joseph was sold into slavery, lied on, overlooked, and thrown into jail.
Yet he still stayed faithful to God.
Eventually he rose to be second in command to Pharaoh over all of Egypt.
His faithfulness lead to his harvest.
Your faith is a seed that will produce a harvest in your life.
Not just one fruit, but many fruits.
Choosing faith over your flesh is the harder thing to do, but it always grants the greatest
reward.
But only when we don't give up.
What are you trusting God for today, Beloved?
Strength to stay pure, a positive attitude when you're not feeling well, the good words
to say when you really want to tell someone off?
Integrity and honor when it's easier to cut corners?
Ultimately, we all have a choice today: to plant a seed of faith and trust God with what
we can't do, or lean on our own strength and place our confidence in our flesh.
Only one leads to the harvest we desire.
But we can rest assured that no matter how hard it is, we will see that harvest if we
don't give up.

Drug companies are taking advantage of people
by overcharging for their drugs.
Congress needs to do something.
I often thought that if my life was going to end early
it was going to end because of something happening in a war zone.
It was not going to end because of cancer.
I was running projects in other countries.
I did things like running elections or anticorruption programs.
I was living in Afghanistan and I feel something and I called my doctor
back in the United States and he said, "You'd better come home
and get an exam."
By this time, the cancer had metastasized.
I now need chemotherapy to stay alive.
My life depends on prescription drugs.
It's very easy for drug companies to gouge us
and demand ridiculous amounts of money
since our lives depend on these drugs.
My biggest fear is that I'm going to run out of money
before I run out of time.
And no one should live that way.
We have to do something about the price of drugs for all Americans.
That's what will drive me until I die.

over the weekend to CBS.
He's apparently determined to work with China to rein in the North Korean regime.
Connie Kim tells us more.
U.S. President Donald Trump's strategic ambiguity regarding North Korea was highlighted during
an exclusive interview with CBS marking his 100th day in the White House.
"We shouldn't be announcing all our moves.
It is a chess game.
I just don't want people to know what my thinking is.
So eventually, he will have a better delivery system.
And if that happens, we can't allow it to happen."
From the start of the Trump administration, the U.S. President dealt with the North Korea
issue as one of the most pressing foreign policy agendas to solve bringing up the card
of "all options" in dealing with Pyongyang.
But issuing a statement on the regime last week, Washington toned down its voice saying
the doors remain open for talks with the North.
In efforts to maximize pressure on Pyongyang, Trump chose to work with China as Beijing
exerts significant influence over the regime.
And by doing so,.. Trump has shown that he is willing to give in to China in trade in
exchange for helping Washington solve the North Korea factor.
"I think that, frankly, North Korea is maybe more important than trade.
Trade is very important.
But massive warfare with millions, potentially millions of people being killed?
That, as we would say, trumps trade."
Following the much anticipated summit between Trump and Xi as well as their subsequent phone
conversations, the two seem to see eye to eye about refraining Pyongyang from launching
a provocation.
"I would not be happy.
If he does a nuclear test, I will not be happy.
And I can tell you also, I don't believe that the president of China, who is a very respected
man, will be happy either."
And seemingly corroborating Trump's statements, the South China Morning Post reported Beijing
would consider an oil embargo for at least six months if Pyongyang pushes the button
on another nuclear test,... while China's ultra-nationalist Global Times reported Beijing
would have little choice but to support harsher sanctions if the regime pushes ahead with
its nuclear ambitions.

Menstrual Cup and IUD / Copper T - Duration: 5:28.
hi everyone today I'm going to do a video on whether you can wear a menstrual
cup with an IUD or a copper t or not. a lot of women have been asking this
question whether they can use a copper t / IUD along with the menstrual cup or not so
it's totally fine a lot of women have been using it successfully without
having any problems however you can end up dislodging your copper T/IUD by using
the menstrual cup so you need to be a little careful. for those of you who
don't know what is a copper t/IUD, it's a birth control device that's inserted in
a woman's uterus to prevent pregnancy. I'll show you a quick demo on how this
works so this is the female reproductive system and that's a woman's uterus so
and this is a copper t/IUD, so it's made of plastic and copper coils if you can see it carefully.
this is inserted in the uterus like this over here see that's
your copper t inside the uterus and it has this string which is sort of like a
plastic thread which comes out of this is your cervix so the strings come out
of the cervix and they are left like this inside the vagina usually that's
the length that you're gyne will leave so that later on when she needs to
remove it she can use this and when you wear the menstrual cup it sits below in
your vagina over here like this so you can see that your thread can be inside
the menstrual cup which can lead to you pulling out the copper T/IUD while removing
the menstrual cup so it's always a good idea to consult your gynie if you're using
both these products together so it will be good to tell your gynae that you will
be using a menstrual cup and she can trim these threads a little short
so when you are wearing the cup they do not interfere and it won't stay
loose inside the cup when you're pulling it out. what would happen is that
if your strings are left long and they are reaching the bottom of the
cup, when you want to remove the cup what we do is we pinch the base and we break
the suction of the cup and we slowly pull it out. so if the strings are
long it's possible that your strings will also get pulled out with the cup
and you will dislodge the copper T/IUD which obviously will not be a very good thing
to do because in that case if you end up dislodging the copper t / IUD you will have to
go to your gynea, get it removed and get a new one inserted again so
it's better to have the string cut short. what I suggest is that after having your
copper t / IUD inserted and string short, you try and keep your menstrual
cup in the vagina as low as possible so now when you strings are short or maybe
even if they are folded up since these are plastic strings you can actually feel
them and you can fold it and leave it like that or pull it out as you want.
it's good idea to always have a feel of your thread so you know how it is when your
copper T/ IUD is in place so after every period when you use the cup and it's
better that you know you can just check that your thread are still how they were
before the cycle the ones both these think that inside there is nothing new
or different that you need to do you just need to be careful pinch the base
and remove the cup your copper T stays in place without any problems at all
there have been some cases where women ended up for dislodging their copper T / IUD
while using the cup so percentage is quite small I'd say one in 20 to 25
women have done that and the possible reasons behind that is one they have a
very low cervix, so this space - the vaginal canal length is quite short so
they end up having the cup and the copper T/IUD quite close to each other and
when they pull it out they dislodge the copper T/IUD. if you have a low cervix and
you want to use the cup along with a copper T/IUD, I will highly recommend going
for shorter length cups. so always measure your cervix and if you know it's
two and a half knuckles you can go for a cup length around that size if it's
one and a half to two knuckles you can go for really short cups like me Luna shorty
another reason that you might end up dislodging your copper T/IUD while
using menstrual cup is that the menstrual cup rides quite high up for
you and then it reaches the cervix. strings of the copper T/IUD can get stuck
inside the cup is the cup has moved to deep inside. so I'll highly recommend
that if you are using the menstrual cups with the copper T/IUD always the get hold of
the base of the cup, pinch it to break the seal, it's always very important to
break the seal of the cups before removing even if you are not using
copper T/IUD, but when you are using it with the copper T/IUD it's a really important
step so that you don't pull out the strings of your copper T/IUD so I hope this
video was helpful and you are able to use copper T/IUD along with menstrual
cup successfully like many other women.

What you find is, if you take the fructose and other natural sugars that are in
the matrix.
Matrix means mother in Latin.
And literally, here's this sugar sitting in the middle of this cell
membrane, and it's got all these phytochemicals around it, and fiber, and it works.
And if you take a different sugar, and take all the matrix away, which is white sugar,
corn syrup, now it's killing people instead of healing people.

My First T Shot (FTM Transgender) - Duration: 7:01.
Hey!
My name's Gray, and welcome to my channel.
So...
I am one day on T. So, in case you're wondering, um what happened was: apparently, my prolactin
being too high wasn't going to affect anything.
It wasn't dangerous.
It wasn't enough to stop me from going on T, so [exhales loudly] yesterday, Monday,
April 24th uh the doctor called and said, "I'm ordering the prescription," basically,
so my mom went and picked it up, and when i got home from school, I gave myself my first
shot.
So what I'm going to do now is just show you the footage of me giving myself my shot um,
and then after I'll talk a little bit about uh how it went, how it lived up to or didn't
live up to what I was expecting.
Uh...
So here you go.

I'm too excited; I forgot what I have to do.
[coughs and laughs] It stinks.

I'm bleeding.
My dosage is .25cc, so it's pretty much the lowest dose I can have at this point um, but
it's still something, so I'm gonna take it and hope that it makes some changes happen.
And I do the shot every two weeks, so we'll see how that goes.
I've heard guy have mood swings like really bad mood swings as um it gets closer to when
they need their shot, but we'll see.
Maybe it'll work out fine.
Alright, so first of all, I didn't know what to expect in terms of pain.
I, I, I was concerned, "Oh my God.
Is this going to hurt every single time I do it?"
And I have to say: It doesn't hurt as bad as you think it's gonna hurt.
It's not comfortable, obviously.
You're injecting yourself in the muscle, but once you get through the skin...
I went super super slow, which probably why it hurt a little bit more, but once youre
through the skin, you can't feel anything.
Once you're through that layer with nerves in it, it goes in so so easily, and you don't
feel a thing.
I mean to be honest, taking the band-aid off this morning hurt just as much, so don't worry
about that.
You're gonna be fine.
So, it wasn't as hard to push in as I thought it was going to be.
I used a 23 guage needle and a one ml syrings, both to draw up and to inject.
Everyone was saying, "It's going to be hard to push in."
I didn't try to push it in fast, which might be why I didn't feel the resistance.
I went super slow with the whole thing.
Injecting, putting it in, like everything was slow.
I'm not the kind of person that's gonna jab myself or force it in really hard.
Like no thank you.
And then taking it out, go fast.
Um, don't go slow because that'll obviously not be as comfortable.
That's it.
This is my voice one day on T, so it's pretty sick.
It's it's awesome.
Obviously, you watch all these videos of other trans guys uh transitioning, and you watch
this three-minute video of them from being pre-T to being one year on T, and that's what
you see of it.
Um , you don't see them one day on T. I don't know like I'm ready for the changes.
I'm like [slaps hand].
I'm impatient with this whole thing, but that's something I'm just going to have to work through
because obviously the changes don't happen immediately.
Nothing's happening today.
Nothing's different today, and it probably won't be for a while, so keep that in mind.
Even once you're on T, you're still gonna have to do some waiting.
It sucks, but it's what happens.
